3. Evidence that services have been effective: An 8 month pilot study conducted by SUNY-Cortland Math Professors on students from the Home Intermediate School in Homer, N.Y.; A 2 year review of E.nopi English program in the Los Angeles Unified School District SES Program.

10a. Description of methods and strategies for serving eligible students with disabilities: Lessons for students with disabilities will conducted at pre-arranged designated handicapped accessible facilities.  Extra time will also be allotted to students when needed.  
10b. Description of methods and strategies for serving eligible students with limited English proficiency: Students with limited English proficiency will receive lessons from tutors that are bilingual in English and their native language.
11. Qualifications of the service provider(s)/instructor(s): Certified teachers with state certificates.
12. Reports to parents, teachers, and LEA (Content, frequency, method of delivery): Will include what students are doing in their tutoring classes, will be sent home and to schools monthly.
13. Will transportation to the service location be provided at no charge if requested? (Note: LEAs are not required to provide or pay for transportation) No transportation will be provided.
