Description of methods and strategies for serving eligible students with disabilities:

The Streamline BR, Inc. utilizes the co-teaching and re-teaching instructional units in inclusive classroom with teachers' shared instructional and program responsibility for a single group of eligible students in a Brooklyn college based classroom setting regardless of students' special needs and number of special needs students in a group; when one teacher delivers the instructional unit while the other provides the appropriate instructional support or addressing individual student’s needs in the same least restrictive instructional setting.

Description of methods and strategies for serving eligible students with limited English proficiency:

The eligible students with limited English proficiency will be served at the learning stations in the same instructional setting when teacher and co-teacher will be delivering the same instructional unit in parallel.

Qualifications of the service provider(s)/instructor(s):

The Master Teachers have the New York State permanent teaching certificates and the individual Teachers have the New York State provisional or transitional teaching certificates (see Attachments, page 7). All teachers and tutors have the Bachelors or Masters Degrees in Math, English, or Education.

Reports to parents, teachers, and LEA (Content, frequency, method of delivery):

The Summary Attendance-Deliverance (SAD) Report will be generated on a weekly basis and delivered to the LEA by e-mail (if requested). The Individual Student Summary (ISS) Report will be generated on a weekly basis and delivered to parents by e-mail or US postal service (if requested).

Will transportation to the service location be provided at no charge if requested? (Note: LEAs are not required to provide or pay for transportation)

The Streamline BR, Inc. does not provide the transportation service.
