Description of methods and strategies for serving eligible students with disabilities:

Small group instruction (2:1).  LHV will match students with tutors with extensive experience working with students with disabilities. LHV will follow student IEP and will create specialized curriculum after discussion with student’s teacher.


Description of methods and strategies for serving eligible students with limited English proficiency:

Small group instruction (2:1).  Student’s will spend 75-minutes on ELA work, with supplemental materials created with after discussion with student’s classroom teachers. LHV will match students with experienced tutors proficient in the language most comfortable to the student.


Qualifications of the service provider(s)/instructor(s):

All Lead Teachers will be NYC DOE certified teachers; all tutors will be college-educated with previous experience tutoring.


Reports to parents, teachers, and LEA (Content, frequency, method of delivery):

Parents – monthly progress reports by mail, one in-person conference each semester.

Teachers – monthly paper progress reports, Site Coordinator will have weekly conversations with teachers

LEA – monthly progress reports by mail.


Will transportation to the service location be provided at no charge if requested? (Note: LEAs are not required to provide or pay for transportation)
