3. Evidence that services have been effective: All of the content and methodologies used by Path to Literacy, LLC are research based.  The reading program is based upon the research of the National Reading Panel (2000).  The mathematics content and instruction is based upon the research of the National Mathematics Advisory  Panel Report (2008).

Small group direct explicit instruction.

Teacher to student ratio 1:1 to 1:8.
10a. Description of methods and strategies for serving eligible students with disabilities:

The student's IEP will be reviewed and all instruction provided  will be  consistent with the student's IEP .  To every extent possible instruction will be provided by teachers trained in special education methodologies.

 
10b. Description of methods and strategies for serving eligible students with limited English proficiency: To every extent possible instruction will be provided by elementary education or content area specialist who have been trained in ELL methodologies.
11. Qualifications of the service provider(s)/instructor(s): New York State Certified Teachers
12. Reports to parents, teachers, and LEA (Content, frequency, method of delivery): The LEA, teachers, and parents will be notified of the students' progress upon the completion of every 10 hours of instruction.  Progress will be provided in writing and a parent/guardian meeeting will be held to discuss student progress
13. Will transportation to the service location be provided at no charge if requested? (Note: LEAs are not required to provide or pay for transportation) No
14. Additional relevant information: We will provide direct explicit small group instruction based upon your student's need that will improve his/her academic ability in reading and mathematics.
