2014-15 NYSAA Sample Verifying Evidence

Below are links to sample verifying evidence in the form of student work samples and data collection sheets in ELA, mathematics, science and social studies. The samples demonstrate how verifying evidence can be designed to assess a student with a severe disability using extensions for English language arts and mathematics and using Alternate Grade Level Indicators (AGLIs) for science and social studies.

Teachers developing NYSAA datafolios for 2014-15 may download and print the samples and use them to assess their students on the NYSAA. While teaches may modify the samples, they must maintain the connection of the verifying evidence to the task being assessed. Assessment task numbers are documented at the top of each piece of verifying evidence. The verifying evidence samples at the link below are not required.
