Program Information Report, 2008-09

General Instructions

All local educational agencies (LEAs) that received Title I funds in 2008-09 are required to complete and submit the 2008-09 Program Information Report (PIR), which addresses academic intervention services.  The report enables the Department to provide Title I program participation data to the U.S. Department of Education.  

PLEASE NOTE:  For this year, NYSED is reducing the data collection burden to only those fields needed for required federal reporting.  Any LEA that did not receive Title I funds in 2008-09 is not required to submit this Form.

There is ONE new category added for this year:  under Services by Subject Area, the subject of Career/Vocational has been added. If you can identify any Title-I funded AIS service provided to students in this Subject Area, enter the Count here.

The usual reporting due date for this Report is at the end of September.  Due to the delay in the posting of this Form, the due date for this year is being extended to November 13, 2009.

All LEAs that are required to complete the PIR are expected to complete the electronic version of the Form and submit it to the Department via the internet.    Do not submit a paper copy of this form. The only acceptable transmission method is via the Internet.

Notes for Completion:

  • To access the form, click on the link above and submit the form online by pressing the "Submit" button at the bottom of the form.  You should receive an acknowledgement.  If you do not receive an acknowledgement ("Thank you for submitting...."), then the transmittal may not have been completed. 

  • If you do not find your LEA listed in the drop-down list, contact the Title I office (phone and e-mail below).

  • Staff FTEs can be reported in decimals (one or two places). Be sure to include the decimal point. All other data are integer values only.

  • When you have completed the PIR form and are ready to submit it to the Department, be sure to print a copy of the form (if desired) and then click on the "SUBMIT" button at the bottom of the last page of the report.

  • Be sure to fill out the " E-Mail Contact Information" field,  and feel free to enter in the "Comments" section any clarifications that you feel are needed as well as comments on the Form itself.

IMPORTANT:  Be sure to include all of the AIS services funded by Title I, Part A in this form.
