New York State Charter School Uniform Application - For Public Comment

Section 2854(2)(b) states that: “Applications for admission to a charter school shall be submitted on a uniform application form created by the department and shall be made available by a charter school in languages predominantly spoken in the community in which such charter school is located.” All reasonable efforts should be made by charter schools to use the uniform application form according to the directions below for admissions. All charter schools must use the uniform application form according to the directions below for admissions in each and every school year.

As part of our ongoing document review process, NYSED is proposing to update the Uniform Charter Application Form and Instructions, which were last updated in 2011. We welcome public comments regarding these updates via email at CharterSchools@nysed.gov with the subject line “Uniform Charter Application Form Comments.” Public comments are due by April 2, 2021. These updated forms will go into effect for use beginning in the 2021-2022 school year.
