2018-19 Continuation Application for CSP Planning and Implementation Grants

Charter schools in year 1 or year 2 of a CSP Planning and Implementation subgrant in 2017-2018 must apply to receive continuation funding for the grant period beginning 7/1/18 and ending 6/30/19. The forms to be completed and submitted are listed below. The first section of forms is needed in order to initiate the 2018-2019 grants for those in year 1 or 2. The remaining sections are needed for those in year 1, 2 and 3 in order to close the 2017-2018 grant period.

Important notice: Effective 2018-19, this grant program is reimbursement only. Once an FS-10 budget is approved, there will be no automatic first payment. On the form FS-25 Request for Funds, requests for anticipated cash expenditures will not be accepted. Careful monitoring of expenditures and prompt submission of FS-25s is essential. FS-25s may be submitted to Grants Finance as often as once a month.

The 2017-18 final expenditure report must also be submitted to NYSED's Grants Finance Office so that the grant can be closed and any final payment can be made. 

Requested by September 30, 2018 from schools with CSP planning and implementation grants ending June 30, 2018:

With the exception of the M/WBE compliance report, please e-mail the documents to the CSP Grants Manager. (The M/WBE compliance report should be e-mailed directly to the Department's M/WBE Unit.) Upon review and approval of the application, a hard copy of the cover page and the FS-10 budget, both with original signatures, must be mailed to the Charter School Office.
