The New York State Education Department has developed a modification application for approved special education preschool providers to seek a change to its currently approved program(s) pursuant to Sections 200.7, 200.16 and 200.20 of the Regulations of the Commissioner of Education. Modification applications may be submitted for multidisciplinary evaluation programs, special education itinerant services, special class in an integrated setting programs and/or special class programs. Agencies and school districts may not implement the proposed modification request until written notification of approval by the State Education Department has been received.
The use of the application to request program modifications is effective immediately. The form is available at http://www.p12.nysed.gov/specialed/applications/preschoolapp/modifapp.htm and includes the Program Modification Request packet, required documentation and instructions for requesting modifications to an existing special education program. Programs seeking a modification must submit one copy of the completed modification request to:
