Grant Application for Individuals with Disabilities Education Act (IDEA) for 2015-16


Materials Required for Application Submission                     

Please Note: Districts may not obligate funds for a proposed federal grant project until the application and budget (FS-10) have been received by the State Education Department (SED) in a substantially approvable form. 

Applicants must submit the following to be substantially approvable.  Please use this as a check list to ensure that all materials have been submitted and are substantially approvable:

  • The Application Cover Page with original signature of the Chief School/Administrative Officer (in Blue Ink)
  • One Option (I, II, III, or IV) must be selected and the information provided for the 2013-14, 2014-15 and current school year's (2015-16) local Maintenance of Effort.  It is recommended that you calculate all Options to determine which choice best reflects your districts Maintenance of Effort.
  • Local educational agency (LEA) Assurance Pertaining to Maintenance of Local Effort
    • Please note if the amount of funds spent each year in the selected Option for the Maintenance of Effort calculation does not increase or remain the same each year, then Maintenance of Effort has not been met and a reason must be selected explaining why on page 5.  Do not select a box if Maintenance of Effort has been met and please note that selecting the “Other” box without providing any information is not acceptable.
  • FS-10 611 and FS-10 619 (if applicable) Budgets with Budget Summary Page with an original signature (in Blue Ink).  Please provide two additional copies of your 611 and 619 grants in addition to these signed originals.
  • LEA Certification Pertaining to the Payment of Funding to Approved Special Education Providers (ASEPs) – Please select ONE option.
  • Coordinated Early Intervening Services (CEIS) Calculation Worksheet.  
  • This is necessary if the LEA is required to (Mandatory CEIS Redirect) or has opted to use up to 15 percent of funds for CEIS (Optional CEIS).  This must reflect funds being spent toward general education to prevent at risk students from being classified as needing special education.  Please note that the items and costs represented on these worksheets must be reconcilable to their corresponding grants.

  • Please check your math on the Indirect Cost Rate (if applicable) as this is a source of frequent error.
  • Please insure that any child reflected in the 619 Grant that is receiving services from ASEPs are also reflected under the 611 Grant as receiving services from those ASEPs.

Prior to copying the FS-10 budget forms, please ensure you have:

  • Included the last four digits of the project number in the space provided on the cover page. Your project number can be found in Column A of the Allocation Table.
  • Checked that the Chief School/Administrative Officer has signed in Blue Ink on the Budget Summary Page.
  • Typed or printed the Chief School/Administrative Officer name and title below the original signature.

The original Application and FS-10 Budgets, (including 2 copies) for the IDEA Section 611 and Section 619 Grants are due by July 1, 2015 to:
