Slide 11: State’s Model IEP form

An IEP is a written statement for a student with a disability that is developed, reviewed and revised by a Committee on Preschool Special Education (CPSE), Committee on Special Education (CSE) or Subcommittee on Special Education, to meet the unique needs of a student. Section 200.4(d) of the Regulations of the Commissioner of Education prescribes what information must be included in an IEP.

The State’s model IEP form is to be used for both preschool and school-age students.

The State’s IEP form may be used in its current Word format, or may be converted to another format, including a computerized format. However, for all IEPs developed for the 2011-12 school year and thereafter, the State’s IEP form may not be modified to otherwise change its appearance or content, with few specific exceptions which are noted on the first page of Attachment 3 — the General Directions for Use.

Schools should insert identifying information on the IEP template to identify the IEP as one developed by their school district.
