Slide 18: Evaluation Results

The results of the initial or most recent individual evaluation of the student, as well as the results of the student’s performance on any general State or district-wide assessment programs must be considered in identifying the student’s present levels of performance and individual needs.

The IEP form includes a section where the instructionally relevant information from a student’s initial or most recent individual evaluation, as well as the results of the student’s performance on any general State or district-wide assessment programs may be documented, to the extent this information is relevant to the current performance and needs of the student.

There is no requirement that the specific names of the individual tests conducted to complete the initial evaluation or reevaluation of the student be indicated in the IEP.
