Prior written notice must inform the parent of each evaluation procedure, assessment, record or report used in the decision to propose or refuse the action.

This example informs the parent that the Committee reviewed existing evaluation information to determine if new assessments needed to be conducted as part of the reevaluation and that this information was discussed with the parent in a telephone conversation.
