Prior written notice must provide the parent with a description of the proposed initial or reevaluation and the uses to be made of the information.

This example informs the parent that by conducting the reevaluation, the Committee will have the information needed to determine the student’s individual needs, educational progress and achievement, ability to participate in instructional programs in general education and continuing eligibility for special education services.
