SEQUENCE OF EVENTS IN THE EVALUATION PROCESS

PRESCHOOL AND SCHOOL-AGE

 

For Preschool Students
Initial Referral for Evaluation

  1. The CPSE chairperson, upon receipt of a referral for initial evaluation, records the date the referral was received by the Committee chairperson or by a professional staff member of the school district, whichever is earlier.

  1. The CPSE immediately sends prior notice to the parent of the student.

  1. The parent sends written consent to the CPSE. If the parent does not give consent for the initial evaluation, the parent is notified that no further action takes place.

  2. The CPSE requests consent of the parent to receive and forward the most recent evaluation report for a child in transition from an early intervention program to the approved evaluator and to the CPSE.

  3. Upon receipt of parent consent and parent selection of an approved evaluator, the board of education arranges for the evaluation by the service provider selected by the parent.

  4. The CPSE, including the parent of the student and the approved evaluator, reviews existing evaluation information on the student (either in a meeting or individually) to determine what, if any, existing evaluation information is current and appropriate to meet the requirements for an initial individual evaluation for this student.

  1. The approved evaluator conducts the evaluations determined necessary.

  2. The approved evaluator:

  1. Prior to the CPSE meeting, the CPSE provides the parent with:

  1. The CPSE chairperson sends a meeting notice to the parent.

  1. The CPSE conducts a meeting to review evaluation results, determine eligibility and, if appropriate, develop the IEP.

  1. The CPSE sends prior notice of the recommendation from the CPSE meeting to the parent.

  1. The CPSE forwards the recommendation to the board of education.

  2. The CPSE chairperson provides a copy of the evaluation report and the documentation of the determination of eligibility to the parent.

  3. The board of education sends notice to the parent when it takes action.

  4. The CPSE forwards a request for approval of reimbursement of the cost of evaluations to the municipality and a copy of the form to the approved evaluator (or a copy of the form available for inspection and photocopying by the approved evaluator).

 

For School-Age Students
Initial Referral for Evaluation

  1. The CSE chairperson, upon receipt of a referral for initial evaluation, records the date the referral was received by the Committee chairperson or the building administrator, whichever is earlier.

  1. The CSE, including the parent of the student, reviews existing evaluation information on the student to determine what, if any, existing evaluation information is current and appropriate to meet the requirements for an initial individual evaluation for this student.

  1. The CSE chairperson immediately sends prior notice to the parent of the student.

  1. If parental consent is not received, the CSE:

  1. Upon receipt of parental consent, the school conducts the evaluations determined necessary.

  2. Either prior to or at the CSE meeting, the school shares the results of the evaluation with the parent.

  3. The CSE chairperson sends a meeting notice to the parent.

  1. The CSE conducts a meeting to review evaluation information, determine eligibility and, if appropriate, develop the IEP.

  1. The CSE sends prior notice of the recommendation to the parent.

  1. The CSE forwards the recommendation to the board of education.

  2. The board of education provides notice to the parent when it takes action.
