State Performance Plan Indicator #8 – Parental Involvement
The New York State Education Department (NYSED) will collect data for the State Performance Plan federal Indicator #8 (parent involvement) from your school district for the 2006-07 school year. The Department has contracted with CyberData, Inc. to assist with this data collection. At this time, you should go to the following website and order your surveys: www.vesidsurvey.com/order. The website will give you a choice of eight languages in addition to English. You will need to have selected your sample and identified the number required in each language prior to submitting your order. Please do not photocopy the surveys for distribution. Surveys provided by CyberData will be in a scanable format.
To determine the number of surveys you will need to order, use the sampling calculator at: http://eservices.nysed.gov/pdsystem/samplesizecalculator.jsp. The eligible population of students from which you must select parents to participate in the survey includes every preschool and school-age student with a disability who is provided special education services in district-operated programs or under contract with other service providers. You may choose to send surveys to all parents in your school district or randomly select a representative sample of parents using the guidelines established by NYSED at http://www.p12.nysed.gov/sedcar/randomno.htm.
Once you have responded and ordered your surveys, CyberData, Inc. will ship the surveys and postage-paid response envelopes to the address you specify. It is imperative that you send the surveys and the response envelopes to parents in your district as soon as possible. It is recommended that you attach a letter to parents when you disseminate the surveys. Attached is a sample letter you may wish to use.
We strongly recommend that you employ a variety of methods to encourage parents to complete the survey. Whenever possible, please encourage parents to complete the survey on the Internet at www.vesidsurvey.com/parent.
CyberData, Inc. will provide a website to update your rate of return. It is your responsibility to take steps to follow up with parents if necessary. All completed surveys must be returned to CyberData, Inc. by August 31, 2007 in order to be counted in this year’s survey.

It is recommended that school districts send a cover letter to parents selected to complete the parent surveys. Following is an example of a parent letter that school districts may use as an attachment to the parent survey.
Our school district is working with the New York State Education Department (NYSED) to collect information on how we help you and other parents of students with disabilities to be involved in their children’s education. Parents from school districts throughout the State are completing the same survey. NYSED is working with CyberData, Inc., an independent research firm, to collect the information.
Your answers to the survey are very important to us. They will help us identify how well we work with parents in order to improve services and results for all our students with disabilities. No one from our school district will see how you answered the questions. CyberData, Inc. will compile the results of the surveys from all our parents and the results for will be available to you in a public report issued by NYSED for our school district.
To complete the questionnaire on the Internet, go to www.vesidsurvey.com/parent. You may also fill out the enclosed paper survey and mail it to CyberData, Inc. in the postage-paid envelope. Please take the time to complete the survey as soon as you receive it.
