The purpose of this toolkit is to provide you with the necessary information to successfully administer your Systemic Supports for District and School Turnaround Grant.  Each section is hyperlinked to the appropriate forms, templates, or websites.  Should you have questions, please contact your STO liaison.

Project Period 2: September 1, 2013 – August 31, 2014

October/November 2013

• Provide Agenda for Visit/Call (61 KB)
• Site Visit/Call Report (139 KB) completed by STO

January/February 2014

• Interim Report (61 KB) completed by district and due no later than 2/28/2014 (56 KB)

March 2014

• Provide Agenda for Visit/Call (61 KB)
• Site Visit/Call Report (139 KB)completed by STO
• Grantee Network Regional Meetings

April 2014

• Obligation Extension Annouced (43 KB) (please note that the final MOU and Budget for 2014-2015 are now due July 1, 2014)

May 2014

• Grantee Network Regional Meetings
• Letter of Intent acknowledging SSDST Obligation Extension intent to apply due May 15, 2014

June 2014

• Fiscal Report (33 KB) completed by district

July 2014

Only if applying for the Obligation Extension complete the following and submit on or before July 1, 2014:

• Final Year 2 Narrative (53 KB) update & Year 3 Budget Summary Chart (158 KB)
• FS-10 for Year 3 (proposed)
• Update Project Plan for Year 3
  

(STO review of Obligation Extension request - notification of acceptance no later than 7/30/2014)

August 2014

• To Finalize SSDST:
  Final Project Report (103 KB) due no later than 8/31/14 (53 KB)
• Best Practices Summary due not later than 8/31/14 (optional) (74 KB)
• Final FS-10F and final Project Plan completed by district
  OR if applying for the Obligation Extension complete the following:
  End Yr 2 Report due by 8/1/13:
  Final Yr 2 FS-10F (compared with original budget)

Project Period 3: September 1, 2014 – June 30, 2015
(only if accepted for an Obligation Extension)

            October/November 2014

• Provide Agenda for Visit/Call (61 KB)
• Site Visit/Call Report (139 KB)completed by STO
• Possible Regional meetings (TBD)

            Jan/Feb 2015
