Directions for Submission of Data to NYSED for 2008-09
Overview: If your school district is scheduled to report during the 2008-09 school year on Indicator #11 (see schedule at https://www.p12.nysed.gov/sedcar/sppschedule.html), your staff will need to collect individual student data throughout the school year 2008-09 to be able to report in fall 2009 through the Student Information Repository System (SIRS) using the Assessment Fact Template. Following is a summary of what is collected, which students you should be collecting data about and when and how to submit the data.
The measure: Indicator #11 measures the percent of children with parental consent to evaluate, who were evaluated within State established timelines. Initial evaluations of preschool children to determine eligibility for special education services must be completed within 30 school days of consent of the parent to conduct the evaluation. Initial evaluations of school-age children must be completed within 60 calendar days of a parent’s consent for evaluation. This indicator is a measure of the extent to which the school district’s Committee for Preschool Special Education (CPSE) and/or Committee for Special Education (CSE) act upon referrals within the timelines.
Data required and for whom: During the year you must collect and be prepared to provide individual student level data, including the date when the student is referred to the CPSE and/or CSE, the date parental consent to evaluate the student is received, the date the CPSE or CSE meeting is held to discuss evaluation results and reasons why evaluations are not completed within the required timelines. Data are to be provided on all students for whom parental consent to evaluate is received between July 1, 2008, and June 30, 2009.
Submission and certification:: Report for each student meeting the criteria noted above. To submit the data, you will use the Special Education Events Template and submit data through SIRS. These data must be submitted to the Level 2 Warehouse and certified in the VR11 Report at http://pd.nysed.gov between September 2, 2009 and November 4, 2009.
