Directions for Submission of Data to NYSED for 2008-09

Overview: If your school district is scheduled to report during the 2008-09 school year on Indicator #12 (see schedule at https://www.p12.nysed.gov/sedcar/sppschedule.html), your staff will need to collect individual student data throughout the school year 2008-09 to be able to report in fall 2009 through the Student Information Repository System (SIRS) using the Assessment Fact Template. Following is a summary of what is collected, which students you should be collecting data about and when and how to submit the data.

The measure: Indicator #12 measures the percent of children referred by Part C (Early Intervention programs) prior to age 3, who are found eligible for Part B (preschool special education services), and who have an individualized education program (IEP) developed and implemented by the third birthday.  This indicator is a measure of the extent to which the school district’s Committee for Preschool Special Education (CPSE) assures that the transition between Part C and Part B services occurs within the required timeline.

Data required and for whom: During the year you must collect and be prepared to provide individual student level data on the dates that children are referred from early intervention (EI) services to preschool special education programs and services, date of parent consent to evaluate, date of the CPSE meeting to determine eligibility, date the IEP is implemented and, for children who do not receive preschool special education services by their third birthdays, the reasons why. Data are to be provided on all students for whom parental consent to evaluate is received between July 1, 2008, and June 30, 2009.

Submission and certification:: Report for each student meeting the criteria noted above. Report the required information by using the Special Education Events Template submitted through SIRS.  These data must be submitted to the Level 2 Warehouse and certified in the VR12 Report at http://pd.nysed.gov between September 2, 2009 and November 4, 2009.  Directions for the 2008-09 submission of data will be posted in the SIRS Manual for 2008-09 at https://www.p12.nysed.gov/irs/SIRS/home.shtml.
