Directions for Submission of Data to NYSED for 2008-09

Overview: If your school district is scheduled to report during the 2008-09 school year on Indicator #7 (see schedule at, your staff will need to collect individual student data throughout the school year 2008-09 to be able to report in fall 2009 through the Student Information Repository System (SIRS) using the Assessment Fact Template. Following is a summary of what is collected, which students you should be collecting data about and when and how to submit the data.

The measure: Indicator #7 measures the percent of preschool children with individualized education programs (IEPs) who demonstrate improved positive social-emotional skills (including social relationships); acquisition and use of knowledge and skills (including early language/communication and early literacy); and use of appropriate behaviors to meet their needs.  This indicator is a measure of the extent to which a preschool child improves in each of the above areas from entry into and exit from preschool special education programs and services.

Data required and for whom: School districts assigned to report data on this indicator for the 2008-09 school year are required to report entry and exit level preschool evaluation results on the Child Outcomes Summary Form (COSF) on all preschool children with disabilities who entered preschool special education since March 2006 and who exited preschool special education between July 1, 2008 and June 30, 2009 after receiving preschool special education programs and/or services for at least six months. Submission and certification:: Report for each student meeting the criteria noted above. Submit data through SIRS by using the Assessment Fact Template to report each child’s level of functioning at entry and at exit in each of the three early childhood outcome areas of the COSF.  Submit data to the Level 2 Warehouse and certify it in the VR15 Report at between September 2, 2009 and November 4, 2009.  Directions for the 2008-09 submission of data will be posted in the SIRS Manual for 2008-09 at
