Reporting Requirements - This information outlines the required reporting process for reporting allegations of child abuse in an educational setting.

Form - This form is completed by school/district staff after the incident has been investigated by the school or district and is submitted to law enforcement and the Office of School Personnel Accountability (OSPRA) at OSPRA@nysed.gov. (See Reporting Requirements above for details.)

There is no deadline for submission, but applications must be accompanied by supporting materials and a $600 application processing fee. Certification is valid for six years. Submit completed Applications for Approval, including the fee made payable to the New York State Education Department to:
