The purpose of this toolkit is to provide you with the necessary information to successfully administer your School Innovation Fund Grant - Rounds 2 and 3. Templates and forms will be hyperlinked, as they become available. For questions, contact your STO liaison via email or at 518-473-8852.

Budget (Updated 6/30/15)
Please discard older versions of (FS-10/Budget, FS-10A/Amendment, FS-25/Request for Payment, & FS-10F/Final Expenditure Report) forms. Begin using the updated forms for all projects beginning with the 2015-2016 program year. The latest versions may be found at http://www.oms.nysed.gov/cafe. These new forms contain updated certification sections.

(Note: A worksheet provided to SIF grantees via e-mail is intended to assist in the collection of information for each SIF-funded school. It cannot be submitted on Review Room. Instead, the report templates found on Review Room must be completed.)

SIF 2 (Only) No-Cost Extension Period (7/1/15-8/31/15) is Available. Due Dates for Participants:

  • April 1st: Email liaison with intent to participate; provide a paragraph explaining plans.
  • May 15th: If participating, please submit the following:
    1. FS-10A (if necessary, to access unclaimed funds and revise for extension period)
    2. Updated Project Plan(s) (Required)
    3. Updated MOU(s) with LP/PC (Required)
    4. Updated MOU(s) with supporting partner(s) (only if applicable to extension period)
    5. Updated Performance Target Chart (only if applicable to extension period)

May 15, 2015: All (SIF 2 and SIF 3) FS-10As Due

  • SIF 2 (all districts whether ending 6/30/15, or participating in extension period)
  • SIF 3 (all districts; budget period ends 6/30/15)

May 22, 2015: SIF 3 (Only) Continuation Plans: The district must submit one plan for each SIF Cohort 3 school.

** SIF 3 Only: The due date has been shortened from 90 days (the usual time frame for federal projects) to 30 days following the end of this project. This due date was reflected on all Grant Award Notices (GANs) that were mailed out to local agencies upon the processing of their 14-15 budgets. This date was provided in the January 13, 2015 Race to the Top Closeout Procedures memo from the NYSED Chief Financial Officer, Don Juron to School Superintendents and School Business Officials. The memo indicates that “Failure to submit the FS-10F Long Form (manual) in an acceptable format by this date may result in loss of funds to the grantee.”
