2011 Annual Visual Inspection Report

During the home stretch for completion of the survey, the system may become sluggish and unresponsive due to the high volume of traffic. If you find you are unable to enter your data, please leave the system and try again later. 1/12/12
There are NO changes in the questions from the 2010 Building Condition Survey to the AVI. When you open up the survey form, you will be presented with blanks next to each question. This is correct. You need only enter data in fields where there is a change in condition from the BCS to the AVI in all fields on the first and last pages (building name, contact info, etc.). Therefore, in order to complete the survey, you will need to refer to your 2010 Building Condition Survey (you will find your completed survey here). You may review the instructions for the BCS/AVI process here. Call Curt Miller at 518-474-3906 if you have further questions.
You may perform the inspection using the 2010 BCS format document (This form corresponds precisely to the online survey form) OR you may simply mark up a copy of your completed 2010 Building Condition Survey document for field use.
Please note: the 2011 Annual Visual Inspection MUST be submitted through the online submission form.

2010 Building Condition Survey in ZIP format. Here you can find a copy of your 2010 BCS and may print this off to use to markup changes in conditions discovered in the field.
BCS aid reminder: Districts that did not submit Building Condition Survey (BCS) Forms to Facilities Planning by 6/30/11 will lose 2010-11 BCS Aid and/or will not be able to claim 2011-12 BCS Aid. 11-01-2011
